If your student is taking a school instrument off campus for use at home, please fill out the "Off Campus" instrument rental form below.   You will need to fill out the portion that says serial number, which can be found either on the instrument (violins and violas) or on the scroll of the instrument (cellos and bass).  If your student's cello or bass has an added PC number sticker, please input the number next to "Property Control".  This is extremely important, as the instruments with a PC number are more expensive, and the auditor checks on their status.

Formal Performance attire for Boys may be ordered from Southeastern Apparel (Poplin Vest Ensemble with Cavalier Pant and Bow Tie Style: ENSF).  Please select the following items in your ensemble:

• Black Poplin vest

• White wing-collar tuxedo shirt

• Black Pleated Cavalier pant

• Adult or Youth black bow tie (depending on student’s neck measurement)

NOTE:  In addition to the ensemble from Southeastern Performance Apparel, your student will need a black belt, black dress socks, and black dress shoes.

Formal Performance attire for Girls may be ordered from Southeastern Apparel (Youth Corelli Dress Style: D4003  Color: black)

NOTE:  In addition to the dress from Southeastern Performance Apparel, your student will need black dress shoes (flat, or with a low heel).
